About Your Office Visit
In order to better serve you and simplify the check-in process, please arrive 20 minutes early to your appointment and bring the following:
- Driver’s License or State Issued ID Card
- Primary and Secondary Insurance Cards
- Referral from your PCP (especially if required by your insurance)
- Current Medication List
- Detailed Past Medical History
- Allergy Questionnaire (if applicable)
- List of questions and/or concerns to address with the doctor
- A copy of any imaging (x-ray, CT scans, MRI etc.), you must bring the actual films, not just the reports
- Past medical records from any other physicians related to the reason for your visit and/or specific to your sinuses, nose, ears, allergies, throat/neck, or lungs
- If you require additional assistance with note taking or translation, please bring a friend or family member to assist you
